TRSAZ Sliding Fee Discount Program (SFDP) Policy
The TRSAZ SFDP Policy SOP ensures that every client has equal access to the opportunity to apply for reduced session fees, based solely on their income and family size.
Definitions:
Income: The total funds from wages, salaries, self-employment, social security, pensions, and public assistance, before taxes or deductions are taken out. Household income includes income received by, or on behalf of, any individual included in the calculation for family size.
Family Size: The total number of client, spouse, and any individuals claimed as dependents on federal tax returns.
Eligibility:
TRSA does not use credit checks, payment history, Medicaid denial letters, asset tests, or “net worth” tests when determining SFDP eligibility for individuals and families with annual incomes at or below 200% of the most current Federal Poverty Guidelines (FPG). Eligibility is determined based solely on the client's household income and family size, as provided by the client on the TRSA Sliding Fee Discount Program (SFDP) Application. This policy applies to insured, underinsured, and uninsured clients, unless prohibited by their insurance. For insured and underinsured clients, the Sliding Scale Income Worksheet can be used to determine the percentage of client’s deductible they will pay, under this policy.
TRSAZ may assist clients who are interested in applying for public insurance, upon request. However, TRSA does not require clients to apply for public insurance, or other financial assistance programs, as a condition for qualifying for, or taking part in, SFDP. TRSA does not require a Medicaid denial letter or deny care based on the outcome of Medicaid’s financial screening assessment. Eligibility is redetermined every 6 months.
Charges:
Non-AHCCCS clients must have a card on file with TRSA. The determined fee will be charged on the day of service. If the card on file is declined, TRSA will contact the client to collect the balance. A client is considered to be engaging in “refusal to pay” if they state, verbally or in writing, that
they will not provide TRSA with the funds requested for their services. This includes, but is not limited to, late cancellation/no-show fees, and may result in discontinuation of services until the balance is paid in full.
Visibility:
To best spread awareness of the availability of this program, it is crucial that this information be presented to clients, and potential clients, at various points of engagement. This policy must be provided, as follows:

Distributed to TRSAZ clients during the intake process, along with the TRSA Sliding Fee Discount Program (SFDP) Application, as part of the intake packet

Download the application below, or ask for one at our office. It is also included in every intake packet. Calculate your monthly household income and family size using the worksheet on the form. Return the completed form to our office. Your discount category is determined from your income and family size, and your reduced fee applies from your next session.
